Rob,
If you have access to your cgi directory, formmail is a good program and will do what you need it to do. I use it on my sites. A number of cgi library sites have it.

If you don't have access to a /cgi-bin, go take a look at
www.cgi-free.com and use their redirected form mailer. It works and the price is great - free. I am using it on a site and have been for a couple of years without so much as a glitch.

If its the html part of it, try www.wnmu.edu/html/forms1.htm

If all else fails I am sure someone(dc) will make you one - reasonably priced of course. : )
